MUST BE COMPLETED BEFORE TRYOUTS OR PARTICIPATION

Complete the steps IN ORDER.

PAGE 1 — CLEARANCE CHECKLIST

☐ 1. SCHOOL INSURANCE
Go to https://www.k12studentinsurance.com

• All athletes need school insurance.
• PK-12 – Low $4, High $7

☐ 2. NFHS COURSES
Go to https://nfhslearn.com/  and complete:

• Concussion FOR Students
• Heat Illness Prevention
• Sudden Cardiac Arrest
• Sportsmanship

Must be completed every school year.

☐ 3. PHYSICAL — FHSAA EL2

Get a physical using the FHSAA EL2 form.

• Valid for 1 calendar year.
• All sections must be completed, including dates.
• Upload Page 4 of 4 only.
• Other physical forms cannot be accepted.

Bring the consent form and EL2 form to the clinic.

 ☐ 4. ECG — NEW HIGH SCHOOL ATHLETES

Incoming 9th graders and first-time high school athletes must have an ECG.

Upload an approved FHSAA EL1.

WhoWePlayFor ECG: Upload the WhoWePlayFor letter AND EL1 with Section A completed.

☐ 5. ATHLETIC CLEARANCE

Go to https://athleticclearance.fhsaahome.org/

• Create an account or log in.
• Use an email you check regularly.
• Enter the full student ID, including 52.
• List ALL sports you want to try out for.
• Upload required documents.

☐ 6. GA4 — SOME STUDENTS ONLY

SKIP if you attended Largo last year OR are an incoming 9th grader.

If you did not attend Largo last year, complete and upload a GA4.
